JOB DESCRIPTION:

AY home care has an immediate opening for Part time and PRN Physical Therapist position for all our service area. We highly value diversity, equity and inclusion and know it matters for our team members and our patients.

 JOB RESPONSIBILITIES:

Complies standards of conduct as set forth by the American Physical Therapy Association.

Practices patient safety and dignity by abiding by Federal, State, Local and AY Home Health Care and procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act).

Develops, documents, implements, and re-evaluates a specific Plan of Care noting all treatment modalities and frequencies.

Treats client/patients to relieve pain and/or develop or restore function to maintain maximum performance.

Reports to the Physician the client/patient’s response to treatment and/or any changes in the client/patient’s condition.

Document clinical progress notes in the patient’s EMR

Supervise Physical Therapy Assistance 

QUALIFICATIONS:

Graduate after successful completion of a physical therapist education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or a successor organization of CAPTE, or a school approved by the American Medical Association (AMA).

Preferably at least one year of home health experience as a Physical Therapist.

Currently licensed in good standing in the state of Colorado.

Must be able to communicate in the language of the client and document client related activity as appropriate. 

Possess a valid state driver’s license and automobile liability insurance 

Mandated by the Colorado Board of Health, you must be fully vaccinated to be considered for this position with AY Home Health Care LLC
