Pre-transplant


Home health nursing care involves providing nursing care and support to a patient in their home while they are awaiting transplant surgery. The nursing care plan will be tailored to the individual needs of the patient and may include the following:

Managing the patient's medications and treatment plan: The nursing team will work with the patient to manage their medications and treatment plan in the weeks leading up to transplant surgery. This may include adjusting medications, monitoring the patient's response to treatment, and providing support to help the patient adhere to their treatment plan.

Providing patient education: The nursing team will work with the patient and their family to provide education about the transplant process, including what to expect before, during, and after surgery. This may include information about medications, follow-up care, and lifestyle changes that may be needed after transplantation.


Post-transplant  


Home health nursing care involves providing medical and support services to patients who have recently undergone a transplant surgery in the comfort of their own home. The goal of post-transplant home health care is to help patients recover from surgery and ensure that they are able to properly care for themselves and manage their medications and treatment plans.

Post-transplant home health nurses to work closely with the patient and their family to ensure that they have a clear understanding of their treatment plan and are able to follow through with all necessary self-care tasks.
